Please contact your category chair if you have questions or issues.
Remember to give plenty of notes to the entrants. The notes you provide on the GOOGLE FORM will be the only notes the entrants receive.

DIRECTIONS
Sign in to your GMAIL before attempting to open the GOOGLE FORM.
If you have trouble, make sure you are using a GOOGLE CHROME browser.
Hover over the small gray box in the top right corner on the form below.
Click the gray box pop-up and open Google Forms.
Fill out the form and click submit at the bottom of the form.
You will receive a copy of your submission in your email in promotions, updates, or spam folders of your Gmail.

Judge Instructions:
Choose whether your scoresheet should be a published or unpublished entry BEFORE opening a Google Form below.
Verify on the entry document and the category chair email when you were assigned the specific entry.
